Position: Physical Therapist (PT)

Overview

Physical Therapist (PT) - Full-Time at The Phoenix Rehabilitation and Nursing Center, part of the Care Rite Centers Network.

The Phoenix is a 400-bed facility with a state-of-the-art Rehabilitation Gym servicing 60+ short-term beds. The Rehabilitation team consists of 30+ Therapists with backgrounds in acute, sub-acute, long-term and outpatient settings. We are a 7-day-a-week rehabilitation facility. In this role, you will have the opportunity to work with a diverse patient population on various sub-acute care services including post-surgical, orthopedic, neurology, post-stroke, cardiac, and much more.

Responsibilities

Responsibilities Include for Physical Therapist:

  • Assess referred patients requiring rehabilitation services and provide direct patient care to evaluate medical condition, functional capabilities, limitations, and rehabilitation potential.
  • Establish and administer a treatment program with specific goals determined by the patient’s capacity and tolerance under the direction of the Physician.
  • Set realistic and achievable goals, document and record the patient’s condition, and educate patients and families in appropriate physical therapy methods.
Qualifications/Requirements
  • Must have graduated from an accredited Physical Therapy program.
  • Must have a valid professional license and registration for the state of NY.
  • Responsible for overseeing and supervising Physical Therapy Assistants.
  • Experience with long-term care and/or geriatric care is preferred but not required.
  • Well-versed with documentation writing; evaluations, weekly progress notes, recertification and discharge documentation.
  • Collaborates with all disciplines to provide patient-centered treatment and care.
  • Exceptional patient service skills and ability to demonstrate clinical excellence.
  • Report to work on time, adhere to scheduled hours, and demonstrate professionalism at all times.
  • Perform other duties as assigned by the Director of Rehabilitation.
  • Compliance with confidentiality, HIPAA and Federal Residents’ Rights.
Compensation

Compensation: $41-$44/hr
