Job Description

The Addiction Consult Service is a multidisciplinary service that provides acute substance use disorder (SUD) care to patients in Emergency Departments (EDs), inpatient (IP) units, and SUD walk‑in clinics, throughout the hospital. The team includes medical providers with addiction training, social workers, peer counselors, and community health workers. Together they provide expertise in medications for AUD and OUD, SUD screening, brief intervention, referral to ongoing treatment (SBIRT), peer support, care coordination, and harm reduction.

Within the Addiction Consult Service, the post‑discharge arm provides intensive, patient‑centered support to high‑risk patients post‑discharge from the ED through ongoing outreach, support surrounding social determinants of health (SDOH), and assistance with linkage to treatment.

Summary of

Essential Duties and Responsibilities
  • As a member of the multidisciplinary addiction consult service team under the clinical direction of the Social Worker III provide addiction counseling to high‑risk patients with substance use disorders post‑discharge from the Emergency Department.
  • Conduct screening, psychosocial assessments, treatment planning, brief intervention, referral to long‑term substance use treatment (inpatient and outpatient) and harm reduction services, and short‑term individual counseling as indicated.
  • Maintain a caseload of clients who require additional clinical support in addition to case management services commensurate with the clinician’s credential level.
  • Provide crisis intervention and support by responding to and following‑through in crisis situations, seeking additional assistance to manage crisis situations as needed.
  • Develop and continually monitor treatment/recovery plans in collaboration with patients and multidisciplinary team members, including establishment of short‑ and long‑term goals.
  • Prepare and maintain confidential case records and files and record patient progress in a timely and accurate manner and in compliance with all applicable standards, to ensure appropriate reimbursement.
  • Participate in evaluating patients’ treatment progress, completing appropriate level of care (LOC) assessments using patient‑criteria tools.
  • Conduct patient follow‑up telephonically and/or in the community, including providing warm hand‑offs to referrals and appointments, aiding non‑clinical team members as needed.
  • Provide opioid overdose education and harm reduction resources including naloxone kits and fentanyl/xylazine test strips to patients and their support systems.
  • Educate patients and their support systems on the etiology and impacts of substance use disorders, treatment options, and the recovery process.
