• Abstract and review patient records for accuracy, completeness, and compliance with ICD‑10‑CM and PDGM guidelines.
• Record documentation deficiencies and reconcile them by notifying responsible individuals.
• Report to supervisor any coding, documentation, or completeness problems that cannot be reconciled.
• Operate PC applications such as HCHB, Swift, and others.
• Participate in special projects, perform additional tasks on a regular basis, and collaborate with CRM and other units.
• Stay current on CMS regulations and coding updates through ongoing education and team training.
• Contribute to a high‑performing, supportive team environment.

• Home Care Coding Specialist – Diagnosis (HCS‑D) certification (Required)
• New hires with Certified Professional Coder (CPC) or Certified Coding Specialist (CCS) will have six months from hire date to obtain the HCS‑D certification.

High School Diploma or equivalency required;
Associate’s Degree in a related field preferred.

Minimum one year of work experience with medical records, including thorough knowledge of ICD‑10‑CM or current coding system and medical record systems. Proficiency with personal computers is required.

USD $36.24 – $36.24 per hour.
