A nonprofit organization is seeking a Case Manager to support elderly tenants transitioning from shelters to permanent housing. Responsibilities include coordinating services, providing advocacy, and helping improve tenants' quality of life through advocacy and support. A Bachelor of Social Work degree is required, along with excellent communication and case management skills. This is a full-time position based in Brooklyn, NY with an hourly pay range of $29.95 - $31.50.
