Position OverviewThe Jewish Board delivers innovative, best‑in‑class mental and behavioral health services to over 45,000 New Yorkers each year. As a Family Nurse Practitioner in our Youth and Families Division, you will evaluate clients for medical conditions, provide treatment, educate on healthy living and disease prevention, and collaborate with mental health clinicians to achieve optimal outcomes. Clinical duties occur in the Supervised Independent Living Program (SILP) for young adults in foster care in the Bronx, and in two group residences:
Kaplan House, Manhattan, and Cedar Knolls House, New York. Most work will be in-person, with some sessions via telehealth.
- Work scheduled hours at two locations:
Kaplan House and Cedar Knolls House. SILP youth will be seen in one of these group residences. Most work will be in-person, but some sessions may be provided via telehealth. - Provide comprehensive primary care services, including detailed histories, physical exams, and diagnoses.
- Incorporate emphasis on preventative care and medical screenings based on established guidance.
- Order and interpret diagnostic tests, using results to formulate treatment.
- Treat acute and chronic conditions, with appropriately scheduled follow‑up and monitoring.
- Make referrals to subspecialist care when indicated, then incorporate subspecialist recommendations into a comprehensive approach.
- Prescribe medication in accordance with best practices.
- Consider a range of treatment modalities and interventions that could potentially improve clients’ conditions.
- Educate clients about their conditions, as well as therapeutic and preventative measures they can use to improve their physical health.
- Utilize motivational interviewing techniques such as SBIRT in addressing behavioral change.
- Remain up‑to‑date with innovations in primary care and nursing practice.
- Understand the interplay between mental health conditions and somatic health, especially the physical side effects often caused by commonly prescribed psychiatric medications.
- Communicate as needed with multiple disciplines—including case managers, social workers, psychiatric providers, and medical subspecialists—in a way that ensures a holistic understanding of the clients’ condition(s).
- Maintain medical records promptly and accurately, utilizing the organization’s Electronic Health Record.
- Handle rotating responsibilities for addressing urgent issues after‑hours.
- Perform initial assessment at admission and annually thereafter (more often when clinically indicated).
- Provide medical health assessment (including differential diagnoses, prognosis), treatment recommendations, and treatment planning for residents.
- Provide health education and counseling about health and sexual/reproductive health.
- Participate in practice improvement projects as initiated by the program and/or the agency Medical Standards and Quality Improvement departments.
- Monitor client progress; recommend discharge and after‑care planning medical needs.
- Communicate needs of youth with program staff and interface with the 29i clinic director.
- Other duties as assigned.
- Skilled provider of comprehensive primary care services.
- Eagerness to provide medical care for clients contending with numerous psychosocial stressors and/or mental illness.
- Comfort establishing rapport with people whose ages span from adolescent to geriatric stages.
- Possess excellent clinical acumen.
- Ability to adapt to changes as the Jewish Board builds primary care components into its service roster.
- Thrive in an environment where continuous operational improvement supports high‑quality clinical care.
- Demonstrated commitment to medical care that centers evidence, person‑centeredness, cultural awareness, motivational interviewing, and harm‑reduction.
- Licensed and Registered Professional Nurse (RN) in New York State.
- Certification as a Nurse Practitioner by the New York State Education Department (NYSED).
