Job Overview
AZAD is seeking a Wound Care Registered Nurse for a W2 Contract position in New York, NY.
Schedule:
9:00 am – 5:00 pm, Monday – Friday. 70% of the role will be in office; nurse may be required to travel to patients' homes in MA, BKLYN, BX & QU to complete wound care consults.
Assignment Duration:
Initial assignment 13 weeks with high probability of extension.
Hourly pay range: $45.00 – $50.00. Benefits include co‑paid medical, dental and vision insurance, cafeteria plan, and 401(k).
Responsibilities- Collaborate with home health interdisciplinary team to review potential wound care patients for Home Care Program and assess eligibility for admission.
- Conduct comprehensive assessment of deep wounds, skin tears, ostomies, severe burns, pressure injuries or bedsores, continence and diabetic foot wounds to determine potential complications and treatment needs; collaborate with team to adjust care as needed.
- Use input from patient, caregiver, physician, and peers to devise a wound/ostomy treatment plan that is feasible within the patient’s physical, financial and emotional resources; create a care plan for the interdisciplinary team, including staff nurses.
- Participate in development, evaluation and revision of all wound and ostomy management policies for CHHA based on evidence‑based trends and industry standards; collaborate with clinical staff and leadership to implement best practices.
- Provide patient and family education and counseling regarding wound care, risk factors, infection prevention and treatment measures; guide personnel in patient care.
- Demonstrate professional attitude and ethical conduct in compliance with departmental standards; maintain effective working relationships with staff, physicians, case managers and the community.
- Write clear and concise progress notes reflecting the plan of care and patient response.
- Provide guidance, education and validation of wound and ostomy skills to field nurses; perform skill evaluations during field visits.
- Coordinate documentation between patients, Home Care Program, medical professionals and referral sources; participate in case conferences and interdisciplinary meetings.
- Collaborate with staff to establish protocols supporting best practices; serve as clinical expert for field nurses and staff training on wound staging.
- Participate in departmental and interdisciplinary conferences, quality meetings and improvement initiatives; collect performance data and metrics for wound patients; review records for compliance.
- Ensure compliance with state and federal home health regulations and agency policies; participate in regulatory surveys; report compliance issues.
- Participate in case‑load reviews and chart audits; develop interventions for identified deficiencies; align care with nursing process and billing guidelines.
- Perform other duties as assigned, including home visits when increased demand or scheduling issues arise.
- Current New York State Registered Nurse License.
- Certification in Wound Care Nurse or Wound Ostomy Care Nurse.
- Bachelor’s degree in Nursing.
- Minimum two (2) years of experience in a certified home health agency or post‑acute care facility; two years of medical/surgical experience required.
- Proficiency with Epic H2O, Remote Client, and Microsoft Office suite.
- Strong verbal, written, organizational and analytical skills.
