We are currently seeking a Per Diem Registered Nurse for immediate hire to join our team in the Bronx and NYC. This is an exciting opportunity for an RN who values flexibility, professional growth, and making a meaningful difference in the lives of patients and their families.

What You’ll Do:- Conduct head‑to‑toe and environmental assessments (initial, reassessment, PRN assessment, supervisory visit) to identify patient‑centered care needs.
- Create a plan of care / medication order based on the assessment and the patient’s needs in collaboration with other healthcare team members.
- Serve as an advocate for the patient to ensure continuity and quality of care.
- Be ready to self‑educate as needed to be proficient in documentation on different platforms and stay compliant.
- Educate patients and caregivers as needed for safe patient care, including preventive care to avoid unnecessary hospitalization.
- Work flexible hours, including evenings and weekends, as needed, to support our patients when they need you most.
- Active RN license and registration (required)
- CPR certification (required)
- Strong communication skills in English (preferred bilingual in Spanish or Russian)
- A calm, professional demeanor and readiness for the de‑escalation of possible obstacles.
- Excellent critical thinking and problem‑solving abilities.
- Readiness for research and implementation of community resources to improve patient care.
