Position SummaryWe have an exciting opportunity to join our team as a Licensed Practical Nurse - NYU Langone Dialysis Center - Mineola, Evenings.
In this role the successful candidate is responsible for using his/her clinical experience to plan the delivery of safe, therapeutic, age‑appropriate patient care through planning, implementation and evaluation over a scope of practice. With the patient and family central to the care, the LPN incorporates the unique characteristics and needs of each patient and family member into the care delivery process.
Works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes. Performs multiple clinical and clerical duties as required. Complies with established policies, procedures, rules and regulations as they pertain to the hospital as well as federal, state and local regulations. Diligently observes all HIPAA guidelines. Performs all other duties as it relates to specialty.

- Complete documentation reflecting multidisciplinary coordination. Document teaching/learning needs of patients and families while in program and in preparation for discharge. Evaluate and document patient’s response to nursing and multidisciplinary team interventions, including patient/family teaching. Evaluate patient’s clinical status and attempt satisfactory resolution of clinical issues at point of care prior to escalating issues. Take action to optimize achievement of patient outcomes.
Participate in identification, investigation and resolution of actual and/or potential risk management, quality care or ethical issues. Address patient issues following established communication hierarchy. - Evaluate patient progress toward health goals and modify critical path or plan of care accordingly. Interact and collaborate with other members of the health team to identify specific patient needs. Initiate appropriate interventions to amend identified variances from plan of care. Document patient and family response to interventions and collaborate with the RN to modify the plan of care as needed.
Provide direct patient care which includes but is not limited to utilizing WUH policy/procedures, patient care guidelines, physician orders etc. Incorporate standards of practice, clinical pathways, age‑specific guidelines and patient care guidelines into plan of care. - Recognize and respond to issues that may cause problems and/or refer issues to the RN; utilize the escalation policy. Recognize the family as defined by the patient and incorporate the principles of family‑centered care in daily practice. Identify patient satisfaction/dissatisfaction with plan of care; elevate issues as required. Consistently verify and document patient advance care directives for assigned patients. Maintain open channels of communication through intra‑ and inter‑shift reporting.
Utilize appropriate communication in all interactions; demonstrate proper telephone etiquette. Actively participate in maintaining a clean, safe, therapeutic environment, including security of patient belongings. Respond to emergency situations as appropriate. Ensure appropriate emergency equipment is present and operational.
