Field Nurse RN Home Care - NYC

Join our dedicated team at Rockaway Home Care, a licensed home care agency serving New York’s downstate area. We are seeking a compassionate and skilled Registered Nurse (RN) to conduct assessments and supervise home health aides in patients' homes. This role is crucial for ensuring our patients receive the highest quality care.

• Conduct comprehensive assessments and aide supervisions in patients' homes.
• Perform thorough head-to-toe patient assessments, including medications and diagnoses, and document findings accurately.
• Supervise and support home health aides/personal care aides in all duties.
• Develop and implement personalized Plans of Care (POC) for aides to follow and provide orientation to the POC.

• Current and valid NYS RN License.
• Ability to travel to patient cases within the 5 boroughs/Nassau/Suffolk.
• Must carry Medical Professional Liability coverage.
• Intermediate proficiency in using computerized systems such as Gmail, Microsoft Word, Excel, and the DOH UAS health care system.

• Bilingual in Spanish/English.
• Experience with Medicaid, Medicare, and Managed Care.
• Experience in home and community-based or long‑term care services.
• Proficiency in using the UAS‑NY application.
• Experience working with vulnerable populations and a passion for patient care.
• Excellent written communication and documentation skills.
