We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide ongoing behavioral health services to residents in skilled nursing and rehabilitation facilities throughout Tompkins County, NY
. This role focuses on continuity of care, routine follow-ups, and collaborative treatment planning in a non-hospital, supportive environment.

• Conduct comprehensive psychiatric evaluations and follow-up assessments.
• Develop individualized treatment plans focused on stabilization and symptom management.
• Prescribe, adjust, and monitor psychiatric medications.
• Communicate treatment recommendations with facility leadership, nursing teams, and families.
• Maintain accurate and compliant documentation in EMR systems.
• Promote resident-centered, trauma-informed psychiatric care.
• Active New York State license as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Board Certified PMHNP.
• Strong clinical judgment, communication skills, and ability to collaborate with interdisciplinary teams.
• Prior experience working in nursing homes, geriatrics, rehabilitation, or long-term care is preferred but not required.
• Experience as a Psychiatric RN or NP is a plus.
