Location: Manhattan, NY (Onsite)
Employment Type: Contract Mon–Fri 09:00am-05:00pm
Compensation:
Pay Range: $50.00-$55.00 Per HR
Description:
• We are seeking a compassionate and skilled Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team.
• This role involves providing holistic, person-centered nursing care to individuals with serious mental illness (SMI) and co-occurring disorders in a community-based setting.
• The ideal candidate is passionate about working with underserved populations and thrives in a mobile, team-oriented environment.

ESSENTIAL DUTIES & RESPONSIBILITIES:
• Provide medication education, administration, and monitoring to ACT clients.
• Conduct comprehensive nursing assessments, focusing on both physical and mental health needs.
• Work closely with psychiatrists, therapists, case managers, and peer specialists to develop and implement individualized treatment plans.
• Support clients in managing chronic health conditions and accessing medical care.
• Offer health education on topics such as medication adherence, nutrition, diabetes management, and harm reduction.
• Participate in daily ACT team meetings and interdisciplinary care planning.
• Respond to crises and provide on-call support as needed.
• Ensure compliance with state and federal regulations, maintaining accurate clinical documentation.
REQUIRED EDUCATION AND EXPERIENCE:
• Licensed Registered RN within the state of New York.
• Must have excellent verbal, written and interpersonal communication skills.
PREFERRED QUALIFICATIONS & SKILLS:
• Experience working with behavioral health and substance abuse clients ideal
• Experience working in ACT, mobile crisis, or forensic mental health settings
