For over 30 years, the Arab-American Family Support Center (AAFSC) has proudly served immigrants and refugees. We help children stay safe, families remain together, and newcomers navigate life in New York City. While our doors are open to all, AAFSC has expertise in serving Arab, Middle Eastern, North African, Muslim, and South Asian communities.

Job Objective

Provide high-quality case planning, work, and management services to at‑risk families referred by the Administration for Children’s Services (ACS). The Case Planner works to prevent foster care placement and ensure child safety and welfare, following ACS standards and the Solution‑Based Casework (SBC) practice model.

Primary

Duties & Responsibilities
  • Ensure timely completion of SBC training & certification.
  • Conduct home and office visits to assess child safety and risk, meeting ACS requirements (minimum twice per month, with additional visits for 0‑2 year olds).
  • Provide case work/case management services to families receiving preventive services (12 cases), including support, goal focus, and family relationship improvement.
  • Complete joint‑home visits within 5 business days of referral.
  • Evaluate family functioning and home safety, formulate outcomes and service plans in partnership with the family.
  • Maintain initial intake documents and process them accurately in the AAFSC internal database (i.e., Apricot).
  • Advocate by linking families to needed community services (medical, mental health, entitlements, day care, etc.) and follow up on participation.
  • Support facilitation of Family Team Conferences, Service Termination Conferences, and other ACS‑led conferences; ensure FASPs are launched 30 days prior to the due date and submitted to supervisor 3 weeks prior.
  • Complete all required documents (progress notes, FASPs, internal forms) within 5 business days, ensuring accurate database entry and record keeping.
  • Maintain relationships with ACS personnel, DCP workers, and FSU workers for ongoing supervision.
  • Collaborate and communicate with PPRS staff, attend supervision meetings, and schedule office days as needed.
  • Attend required training (minimum 42 hours per fiscal year) and log certifications.
  • Assist in preparation of internal and external audits related to program operation and service delivery.
  • Perform other job‑related duties as assigned.
Background &

Position Requirements
  • Bachelor’s degree from an accredited college or university.
  • Familiarity with PROMIS and Connections.
  • Computer skills in Microsoft Office suite and excellent writing skills.
  • Bilingual or multi‑lingual fluency required;
    Arabic, Bangla/Bengali, Hindi, or Urdu skills strongly preferred.
  • Experience working with under‑served students in urban schools, child welfare system, or community‑based settings a plus.
  • Discretion regarding sensitive personal and legal information.
  • Job contingent on pre‑employment background check and fingerprint clearance.
