Social Care Services Navigator; Anticipated

Delaware Opportunities, Inc. is seeking 2-full-time Social Care Services Navigators for our Hamden location. Pay range to start at $22.93 per hour, 35 hours per week. The ideal candidate must be hard working, reliable, and trustworthy.

Responsibilities include:

Conduct screening services and provide confirmation of unmet health related social needs, refer and assist in navigation to services. Determine eligibility simultaneously for any other Delaware Opportunities services and make appropriate referrals to enhanced health related social need services, both internally and externally. Develop a Social Care Plan for each member who is deemed eligible for services. Comply with Code of Conduct and Conflict of Interest policies of both Delaware Opportunities as well as Care Compass Collaborative’s Social Care Network.

Participate in required trainings; perform any similar duties as required or directed.

Skills required are:
Good communication and organizational skills. Non-judgmental. Attention to detail. Ability to meet clients both in an office setting and locations other than the office (home visiting). Ability to work as a team and independently.

Candidate must possess a valid NYS drivers license, and a high school diploma and/or 2 years experience in Human Services.
