Compensation Range: $23/hr - $26/hr commensurate with experience
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. Overview:
The Housing Placement Specialist will serve as the primary connection between clients, case management staff, service providers, HIV/AIDS Service Administration/HASA case managers, landlords/property managers and brokers. The Housing Specialist works with clients in the Ryan White HPC housing placement program and serves as a role model and advocate in the areas of entitlements, self-advocacy, empowerment, health home, behavioral health, and housing.
Responsibilities:
- Attending information meetings to remain current on trends and entitlement system change.
- Assisting individuals and families with finding affordable housing.
- Developing relationships with landlords/brokers and real estate managements
- Track housing statistics and effectiveness of stably housing individuals that site housing as a barrier to achieving positive healthcare outcomes.
- Serving as a liaison between community partners, property managers and other participants to secure housing opportunities for consumers.
- Escorting clients to apartment viewings
- Conducting HQS inspections
- Tracking, monitoring, and reporting housing placement rates for all consumers.
- Assessing barriers to housing individuals and families who are experiencing homelessness to determine housing and service needs.
- Bachelor's degree in human services, Social Work and Psychology
- At least 2 years of experience working with populations diagnosed with chronic health conditions, HIV/AIDS.
- 2 years or more of housing experience.
