- Conduct thorough assessments of children and families to identify strengths, needs, and goals.
- Collaborate with school personnel and other stakeholders to gather relevant information and develop comprehensive support plans.
- Utilize evidence-based practices and assessment tools to inform decision-making and goal-setting.
- Coordinate and facilitate access to a range of support services, including educational, mental health, medical, and social services.
- Act as a liaison between the school, families, and external service providers. Monitor service delivery and progress towards goals.
- Support, advocate and educate for the needs and rights of children and families within the school and larger community.
- Documentation and reporting
- Performs other related duties as assigned.
- THIS IS A FIELD POSITION. Not in office or remote.
- Bachelor’s Degree in Social Work, Psychology or Social Services.
- At least 2 years’ experience in related field.
- Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.
- Proficiency in Microsoft Office.
