We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. Within the Health Home structure, the Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children and families with complex needs. The Care Manager develops and implements a patient-centered, comprehensive Plan of Care for the member, in partnership with his/her caregivers and providers. The Care Manager also is a central point of contact for the family and child, to ensure that their needs are being met, their voices are heard, and their needs and preferences are integrated into plans of care.
Likewise, the Care Manager is the key link among all service providers for each family, working to ensure that all providers are operating with complete, current and consistent information, and that they are aligned in the goals they each are pursuing with the family and child.
A Casework Supervisor provides support and supervision to assigned staff including Caseworkers and Socio‑Therapists as they work with children and youth, their birth families and their foster families.
Required Qualifications- Master’s Degree in social work or related field
- 2+ years of related work experience as a supervisor in child welfare
- 2+ years of foster care work experience
- Knowledge of and/or training in child development stages
- Knowledge of and/or training in family dynamics
- Excellent communication skills (verbal and written)
- Proficiency in Microsoft Word and Outlook
- Ability to engage with clients
- A valid driver’s license is strongly preferred

- Overall responsibility for overseeing all aspects of assigned cases
- Hire, train, supervise and evaluate all assigned staff.
- Coordinate monthly staff meetings
- Assist caseworkers in developing realistic treatment goals for the children on their caseloads
- Ensure assigned staff conduct Field Visits to client’s homes, schools, treatment centers, etc
- Ensure monthly foster home visits to assess the quality of care, well being, and safety in the child’s home environment.
