This position is located within the NYC Health + Hospitals system’s Outposted Therapeutic Housing Units (OTxHU) program. OTxHU provides high‑quality clinical care to patients in custody who have complex health conditions, bridging the gap between jail-based care and inpatient hospitalization. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will open as early as the end of 2024.

Duties & ResponsibilitiesUnder the supervision of the Site Medical Director, the Physician will provide comprehensive, compassionate care to patients with complex chronic disease in the New York City jail system.
Responsibilities include:
- Diagnose and treat acute and chronic illnesses; evaluate and submit priority consults.
- Complete comprehensive histories and physicals on all new admissions, document problem lists, diagnoses, orders (e.g., labs, imaging, referrals) and medications.
- Evaluate patients requesting sick call, schedule follow‑ups, update medication orders and reconcile patient orders at all visits.
- Implement plans for patient care using approved protocols and standard of care treatment plans.
- Request and interpret radiology, labs, EKGs when clinically indicated in consultation with supervisors.
- Collaborate closely with CHS Physician Assistants, including providing clinical guidance, cosigning notes, and offering supervision as required.
- Review all specialty consults and hospital returns to ensure standard of care and implement consultant recommendations.
- Perform chart reviews and summaries for patients transferring facilities, updating problem lists and medication orders.
- Generate special‑needs referrals and documentation for patients with disabilities, dietary restrictions, heat sensitivity, or other relevant flags.
- Teach patients about their medical conditions and treatments; counsel on risks, benefits, and alternatives; document patient refusals.
- Ensure all progress notes and orders are signed before shift end.
- Respond to emergencies promptly and professionally.
- Notify appropriate parties, including Urgicare, about 3‑hour runs and EMS activation.
- Complete special housing rounds when assigned.
- Maintain quality of care and population health indicators; take appropriate action to meet or exceed standards.
- Maintain clinical competency through CME and CHS training requirements.
- Maintain punctuality, timely notification of absences, and adherence to policies and procedures.
- Complete tasks delegated by Site Medical Director or supervising clinical team member.
- Maintain required credentials and licenses.
- Adhere to Occupational Health Services requirements.
- Graduation from an approved medical school.
- Completion of approved residency or fellowship in the specialty or sub‑specialty; board eligible or certified or sub‑board eligible or certified.
- Five years experience in the field of specialty or sub‑specialty acceptable to the Medical Board of the Hospital.
- Licensed to practice medicine in the State of New York.
- Three to five years’ work experience relevant to the specialty.
- Experience working with patients in skilled nursing or residential settings.
- Experience with patients who have serious mental illness.
- Experience with patients carrying substance use diagnoses; knowledge of harm‑reduction approaches and medications for opioid use disorder.
- Experience leading quality improvement initiatives.
- Understanding of trauma‑informed care.
- Skilled in patient‑centered shared decision making.
- Skilled in communicating risks and benefits of clinical interventions and assessing capacity for informed decisions.
- Completion of residency in internal medicine, family medicine or other primary‑care oriented specialty.
- Compliance with Maintenance of Certification requirements or other Board Certification requirements.
- Excellent interpersonal communication skills and ability to collaborate within multidisciplinary teams and with NYC DOC staff.
