Job

Salary: $239,000 annually

• Provide medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care;
• Coordinate behavioral health services for individuals with maladaptive behaviors;
• Carry a patient case load of 100-120 residents;
• Participate in cross coverage for absences of other team members;
• Participate in swing shift coverage onsite, including weekends and off-site telephone coverage on a rotational basis;
• Perform medical assessment for new admissions and readmissions and write orders for required medications, treatments, diagnostic studies and other necessary care and treatment;
• Perform required physician visits based on resident need, facility policies and regulatory requirements;
• Write timely orders and progress notes;
• Initiate referrals for consultant and specialty services as required and write orders for services needed;
• Write orders for rehabilitation services;
• Participate in advance directives planning with the resident and representatives and write related orders;
• Facilitate the emergency discharge of residents to acute hospitals as well as planned discharge for medical treatment;
• Participate in interdisciplinary rounding and meetings;
• Respond timely to calls from residents and their representatives to discuss the resident’s medical care;
• Provide medical leadership for team coordinated Quality Assurance Performance Improvement initiatives;

• Current New York Medical License;
• Board certification in Geriatrics;
• Strong communication, teamwork, and leadership skills preferred;
• Clinical and administrative experience preferred;
