Salary: $239,000 annually
Clinical Role and Responsibilities- Provide medical management for geriatric and non-geriatric nursing facility residents including primary care, management of acute illness, and transition of care;
- Coordinate behavioral health services for individuals with maladaptive behaviors;
- Carry a patient case load of 100-120 residents;
- Participate in cross coverage for absences of other team members;
- Participate in swing shift coverage onsite, including weekends and off-site telephone coverage on a rotational basis;
- Perform medical assessment for new admissions and readmissions and write orders for required medications, treatments, diagnostic studies and other necessary care and treatment;
- Perform required physician visits based on resident need, facility policies and regulatory requirements;
- Write timely orders and progress notes;
- Initiate referrals for consultant and specialty services as required and write orders for services needed;
- Write orders for rehabilitation services;
- Participate in advance directives planning with the resident and representatives and write related orders;
- Facilitate the emergency discharge of residents to acute hospitals as well as planned discharge for medical treatment;
- Participate in interdisciplinary rounding and meetings;
- Respond timely to calls from residents and their representatives to discuss the resident’s medical care;
- Provide medical leadership for team coordinated Quality Assurance Performance Improvement initiatives;
- Current New York Medical License;
- Board certification in Geriatrics;
- Strong communication, teamwork, and leadership skills preferred;
- Clinical and administrative experience preferred;
