Parker At Your Door, a medical house calls program serving the greater New York Metro region, is seeking a Primary Care Physician (MD/DO) to provide quality primary medical care to our patients in the comfort of their homes.

Position Overview

As a Primary Care Provider, you will assess members with multiple comorbidities residing in the community. You will perform comprehensive evaluations of patients and provide medical management.

Responsibilities
  • Examine patients and order appropriate diagnostic workups
  • Assess members with acute condition changes promptly
  • Maintain complete and accurate medical records
  • Collaborate with interdisciplinary team to develop care plans
  • Educate patients on disease processes, medications, and compliance
  • Manage patient medical care
Qualifications
  • Licensure and current registration to practice as a Physician in New York State
  • Strong communication and follow-up skills
  • Proficiency with Microsoft Office, including Excel, and experience with data entry, documentation, and report generation
  • Experience with the geriatric population and home care preferred
  • Knowledge of medical documentation
