Position OverviewAs a Primary Care Provider, you will assess members with multiple comorbidities residing in the community. You will perform comprehensive evaluations of patients and provide medical management.

- Examine patients and order appropriate diagnostic workups
- Assess members with acute condition changes promptly
- Maintain complete and accurate medical records
- Collaborate with interdisciplinary team to develop care plans
- Educate patients on disease processes, medications, and compliance
- Manage patient medical care
- Licensure and current registration to practice as a Physician in New York State
- Strong communication and follow-up skills
- Proficiency with Microsoft Office, including Excel, and experience with data entry, documentation, and report generation
- Experience with the geriatric population and home care preferred
- Knowledge of medical documentation
