Primary Care Provider

The Primary Care Physician (PCP) demonstrates accountability for outcomes, coaches patients toward healthier behaviors, and simplifies care to enable actionable changes. The PCP works independently and with other health care professionals to ensure high-quality, cost-effective care for a patient panel of up to 450 individuals.

• Independently provide care for patients with acute and chronic illnesses encountered in the older adult patient population.
• Take full accountability for patient care and outcomes and seek appropriate consultations from specialists when needed, while maintaining involvement in detailed patient care.
• Engage with hospitalists for patients in the hospital, regardless of whether the hospitalist is employed by Chen Med.
• Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute and complex chronic health needs.
• Lead the care team (care promoter, care facilitator, care coordinator) for patients who can visit the office.
• Coordinate care for patients who are unable to come to the office (in hospital, SNF, LTC, or homebound) with the transitional care team and other resources as available by market.
• Active role in center management and coverage for other providers as needed; participate in recruiting patients and providers as required.
• Perform other duties as assigned and modified by management.

• Proficiency in Microsoft Office and related software as used by the company.
• Willingness and ability to travel locally, with initial training (2 weeks) and occasional national travel for recruiting or training as needed.
• Fluency in English (verbal and written); other language requirements may be noted at the time of application.
• Ability to exercise independent judgment.

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (required).
• Active or eligible for a State Medical License in the state(s) where you will work.
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred. Board eligibility is required.
• Maintain board certification through MOC, CME, and/or retaking board exams as required after becoming board certified.
• Current DEA number for schedule II-V controlled substances.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.
