Job Description:
The Transition Specialist will work with Cleary’s students to prepare for
success in internship experiences and life after high school, including employment,
postsecondary education, and independent living. Lessons will include work skills, resume
writing, interview skills, and overall facilitation of transition beyond High School.
Role and Responsibilities (NYSED)- Transition Planning:
Develop and update the transition components of a student's Individualized Education Program (IEP), which legally must begin by the time the student turns 15 (and earlier if appropriate). - Assessments:
Conduct age-appropriate transition assessments to identify each student's strengths, preferences, interests, and career goals. - Work-Based Learning:
Coordinate job shadowing, internships, and vocational training opportunities in partnership with local businesses or programs like ACCES-VR Pre-ETS. - Agency Linkage:
Connect students and families with adult support services, community agencies, and higher education resources.
The teacher will also provide students with the academic support they need to be successful in certain courses in which they are enrolled.
Minimum Qualifications:- NYS Teacher of the Deaf or Special Education Certification
- Experience working with special needs students, particularly deaf and hard of hearing students.
