Alvita Care is a mission-driven Home Healthcare & Personal Care Agency with four branches and over 500 caregivers. We provide relief and assurance to family members in the areas of In-Home Personal Care, Alzheimer's & Dementia Care, Palliative & Hospice Care, and Geriatric Care Management. We have earned The Joint Commission Gold Seal of Approval. All clients are private-pay and we do not accept Medicare or Medicaid.
As we expand our circle of care, we are seeking top-notch caregivers to service our Westchester and Rockland clients. We have private-duty one-on-one care cases available as well as facility-based care cases.
All applicants are subject to a phone screening to determine compatibility and overall availability of accommodations for both the caregiver and the agency. Cases with Alvita Care are only available to caregivers who successfully complete onboarding.
Responsibilities- Assess client needs to develop a plan of care; monitor and adjust the plan as required.
- Consult with Management on complex or unique cases; provide care plans with support from multidisciplinary team members.
- Ensure services follow the plan of care through assessment, planning, coordination, teaching and supervision.
- Monitor patients/clients' condition and the effects of the care plan; oversee care provided by Licensed Practical Nurses and Paraprofessionals.
- Manage, plan, coordinate and execute the patient’s safe discharge from service.
- Ensure physician orders and discharge communications adhere to company and regulatory requirements.
- Communicate with the patient's MD as needed and coordinate all aspects of the assigned caseload.
- Assist patients with activities of daily living and document all services in the internal system.
- Perform other related duties and participate in special projects as assigned.
- NYS RN licensure.
- BLS/CPR/ACLS/PALS certification.
- Minimum of one (1) year demonstrated experience as an RN.
- Ability to put patients at ease and explain physician's instructions.
- Ability to multi-task with excellent organization skills.
- Experience working with people from diverse racial, ethnic and socioeconomic backgrounds.
- Knowledge of medical terminology.
- Team player with the ability to work independently.
- Valid driver's license or NYS Non-Driver is required.
