Responsibilities
  • Evaluates the degree of function and disability of patients, according to professional judgment and physician orders.
  • Develops a treatment plan based on patient evaluation and physician orders, to maximize independent, functional mobility.
  • Performs physical therapy procedures as ordered by a licensed physician.
  • Records physical therapy evaluations, re-evaluations, progress notes, consultations, and discharge summaries within policy timelines; completes documentation by the end of the day of the visit.
  • Performs the Comprehensive Assessment (OASIS) at appropriate time points, and provides aide orientations and biweekly supervisions.
  • Assesses the need for adjunct services (e.g., SLP, OT, MSW, aide, skilled nursing) to achieve patient outcomes.
  • Educates patients and families in the therapy process and treatment.
  • Participates with other personnel in formulating the patient’s total plan of care and collaborates with the clinical team.
  • Consults with the physician regularly regarding the therapy plan of care and patient progress.
  • Develops or assists in the development of physical therapy policies and procedures.
  • Assists in the instruction of agency staff as needed.
  • Performs other appropriate duties as needed.
Other Responsibilities
  • Timely and accurate documentation as evidenced by clinical record audits and submission of required documentation.
  • Follows infection and exposure control protocols.
  • Attends required agency and departmental staff meetings and meets with direct supervisor as scheduled.
  • Maintains current knowledge of therapy practice; participates in in-service training programs.
  • Participates in quality assurance and quality improvement programs.
  • Complies with agency policies and procedures and follows the Code of Conduct.
  • Undertakes special activities as assigned and other duties as appropriate.
Qualifications
  • Graduate of an accredited physical therapy program.
  • Licensed and currently registered by the New York State Education Department as a Physical Therapist.
  • At least two years of supervised clinical experience.
  • Understanding of the principles, techniques, and practices of physical therapy.
  • Understanding of operation and maintenance of physical therapy equipment.
  • Understanding of conditions and socioeconomic factors affecting patient disabilities.
  • Competent clinical skills demonstrated by a skills checklist at hire and every three years thereafter.
  • Current NYS driver’s license and reliable transportation to/from work assignments.
  • Familiarity with medical database systems.
  • Immunization records and physician health statement at hire and after illness-related absences of more than 3 days.
  • High degree of confidentiality required.
Physical Requirements
  • Lifting, bending, standing, and transferring patients (with equipment when necessary).
  • Mental/emotional demands including handling grief upon patient decline or death.
  • Requires computer use for a minimum of 25% of work hours.
  • Home care field work involves travel to patients’ homes and may involve driving in inclement weather.
  • Occasional irregular hours; flex schedule as needed for patient welfare.
  • Exposure to infectious diseases; follow established protocols.
