DESCRIPTION:

Scatter Site Case Managers are responsible for intensive case management services to consumers utilizing continuous assessment, monitoring, evaluation of consumer health, mental health status and daily functioning. Case Managers shall provide coordination and care that is safe, timely, effective, efficient, equitable and client centered. Case Managers will maintain a caseload of between 15-20 consumers and shall be required to engage consumers in a minimum of one in-home visit, one in-office visit, and one phone call (text) wellness check each month.

Case Managers are accountable to the Clinical Coordinator/Supervisory Case Manager.

RESPONSIBILITIES:

  • Adhere to professional ethics promulgated by training, industry standards, HRA/HASA and HCCI policy and procedures
  • In collaboration with the CC/SCM and the consumer develop services plans (long/short range goals) and update every six months
  • Monitor regular medical follow-ups; annually update physical examination and laboratory reports
  • Document consumer engagement events and other significant activity, in case progress notes
  • Conduct initial and update annual psychosocial assessment
  • Investigate incidents and other reports as appropriate
  • Participate in the initiate intake assessment of HRA/HASA referrals
  • Complete an initial assessment of consumer functioning and needs
  • In conjunction with the CC/SCM develop a Service Delivery Plan with the consumer and his/her partner or family, when appropriate
  • Refer consumers for support services and act as a liaison
  • Provide consumers with assistance obtaining entitlements
  • Help manage medical and personal emergencies such as (need forhospitalization, physical/mental health decline, suicide threat or physical abuse)
  • Act as a liaison for a resident's relationship with family members, care partners, significant others at the request of the consumer
  • Attend and participate in case conferences, meetings, and educational/training seminars as required
  • Escort consumers to other service providers as needed
  • Monitor consumer charts in accordance with HCCI's Support Services Policy and Procedures
  • Counsel consumers of their payment of rent and utilities in the program
  • Maintaining professional working relationships with HIV/AIDS Service Administration case management staff
  • Continuously advocate on behalf of the consumer
  • Conduct home visits, in-office visits and/or other engagement contacts, on a continuous basis
  • Encourage a trusting relationship with consumers to monitor physical/mental health capacity for independent living
  • Work with significant other/family when consumer passes away with referral for bereavement counseling and relocation efforts.
  • Perform other duties and responsibilities required by the Clinical Coordinator/Supervisory Case Manager or the Program Director of Scatter-Site Housing

REQUIREMENTS:

  • Must have a valid driver’s license.
  • Bachelors Degree, or AA, or CASAC, with 5 years’ experience in case management, homeless services, street outreach, substance abuse and/or mental health treatment required
  • Minimum of 3 years’ experience in a mental health setting working with serious mentally ill, substance abuse and homeless population and working in a community residence, supported or supportive housing
