The community you'll be caring for: 701 W PLYMOUTH AVE, Deland, 32720

Responsible for promoting the psychosocial health and wellbeing of patients through assessment and coordination of resources. This position facilitates communication between patients, families, and healthcare and community resources. Additionally, this role collaborates with interdisciplinary teams and external stakeholders to provide comprehensive care coordination. In addition, this position addresses social determinants of health to eliminate care gaps. This role coordinates a wide range of community-based and healthcare support services for patients.

• Other duties as assigned.
• Coordinates with interdisciplinary teams to ensure comprehensive care.
• Develops and implements care plans that address social determinants of health.
• Maintains accurate and up-to-date patient records and documentation.
• Provides ongoing support and follow-up to ensure continuity of care.
• Provides individual and family counseling to address emotional and psychological needs.
• Facilitates support groups to connect patients and families with others facing similar challenges.

Minimum qualifications
:

• Bachelor's in social work or related mental health field [Required]
• 2 experience [Required]
• 2 of social work or related mental health experience, preferably in a healthcare setting [Required]

Preferred qualifications:

• Care management experience [Preferred]
• Work in a community setting such as home health, hospice or other related setting [Preferred]
