Under supervision, manages medication, provides health and wellness support, collaborates with staff to facilitate client wellness, maintains confidentiality, and supports a trauma‑informed care environment.
Essential Duties- Provides medication management within the residential setting.
- Administers medication to clients in accordance with authorized prescriptions.
- Documents medication administration in each client’s Medication Administration Record.
- Logs received medications for each client within the facility.
- Tracks client compliance with required medications and reports concerns to the clinical team.
- Coordinates shift coverage with the Residence Manager for unplanned absences.
- Supports client wellness day‑to‑day and identifies potential medical concerns.
- Facilitates communication and collaboration with agency staff, medical providers, and community organizations to coordinate client care and services.
- Completes required paperwork related to communication or services provided.
- Assists medical staff and prescribers and assists with vital signs.
- Collaborates with residential staff on client discharge plans and participates in staff meetings, clinical supervision, and agency trainings.
- Engages in professional development through supervision, in‑service training, and conference attendance.
- Demonstrates awareness and active support of the Agency’s Mission, Vision, Values, and Strategic Plan, including core competency initiatives of collaboration, community‑focus, innovation, entrepreneurship, and cultural competence and diversity.
- Other duties as assigned.
- High School Diploma or GED and accredited LPN diploma required.
- Valid Licensed Practical Nurse required.
- Two to three years of experience in nursing services to individuals with mental health or substance use disorder preferred; bilingual in Spanish preferred.
Physical Demands/Work Environment:
The position requires frequent sitting, standing, walking, using hands to type, and light lifting. Reasonable accommodations available for individuals with disabilities. Noise level is usually quiet.
