Reports to: PINS Supervisor
Employment Status: Non-Exempt/Full Time
Salary Range: $50,000 – $55,000
Qualifications- Master’s level social worker, mental health counselor, marriage and family therapist or related clinical degree.
- Minimum of one year of post graduate direct service counseling work, or previous case management/internship experience(s) may be substituted.
- A commitment to human services as a profession.
- An understanding of and commitment to helping profession ethics.
- Is culturally competent.
- Ability to work independently.
- Demonstrated ability in public speaking and program delivery.
- Current and valid NYS driver’s license.
- Assures high-quality client services.
- Provides direct service to clients – individuals, children, and families.
- Provides counseling to individuals, children, and families through individual and family counseling.
- Initiates both internal and external referrals to providers/agency services.
- Coordinates with referral and agencies.
- Develops, plans, and coordinates support groups with the PINS team.
- Maintains relationships with Herkimer County and with other referral sources.
- Updates job skills through professional development as needed.
- Contributes to team effort by performing other duties as assigned or those duties requested by supervisor.
- Maintains accurate client records and statistics.
- Maintains confidentiality of all clients.
- Abides by their affiliated Code of Ethics, such as the National Association of Social Workers.
- Maintains and promotes positive work environment.
- Assist families to explore community-based resources, services, support groups, and learning opportunities.
- Assist families to enrich family relationships and rebuild natural support systems.
- Collaborate with providers, referral sources and community supports. Coordinate, recruit as needed and facilitate family support groups.
- Make presentations to community groups.
- Participate in regular staff meetings and supervision.
- Complete documentation in a timely manner including assessments, demographic information, treatment plans, progress notes, etc
- Provide Programming to families – one on one and in group setting
- Establish and build relationships with community organizations and resources relating to the agency Mission/Vision.
- Attending local, regional and state committee/consortium/coalition meetings as relevant.
- Demonstrate sensitivity to, and respect for, the diverse cultural backgrounds and practices of volunteers, consumers and staff.
- Compile program statistics including monthly, quarterly and annual reports.
- Provide training to volunteers and referring professionals when needed.
- With the FPA assist in weekly family-friendly events throughout Herkimer County.
- Collaborate with Peers in other parts of the state especially through CFLR
- Participate as needed with the Peer Family Support Group.
- Provide material for website/Facebook postings and calendar updates.
- Attending CFLR staff meetings and other agency functions as required.
- Provide occasional transportation of program clients in accordance with CFLR’s transportation policy.
- Abide by HIPPA laws and State and County PINS Diversion regulations.
- Any other duties/tasks assigned by supervisor.
