Mobile Unit Independent Living Specialist

Employment Type: Full-Time

Overview

Summary:

Responds to requests and inquiries from the community members and people with disabilities by providing information and referral. Conduct independent living evaluations for people with disabilities, at Mobile Unit stops, in the office and during home visits, to assist them in identifying their needs and goals. Working collaboratively with the disabled individual, develop an Independent Living Plan (ILP) which documents the services and supports they will receive to assist them in meeting their needs and achieving their goals.

Inform individuals about various services and supports that are available to them and assist them in securing and/or maintaining benefits. Coordinate and/or provide independent living skills training to persons with disabilities. Coordinate and/or provide peer counseling to persons with disabilities, both as a one-on-one service and through peer support groups. Coordinate or provide advocacy on behalf of people with disabilities as part of their ILP and, at the request of the consumer and under the direction of the Manager and Director, intervene in crisis situations.

Coordinate or provide support to individuals with disabilities to:
1) move out of nursing facilities and other institutions into independent lifestyles of their choice;
2) prevent them from entering institutions; and
3) transition from high school to adult life including next steps such as going to college or obtaining employment. Advance Independent Living and the rights of people with disabilities through systems advocacy under the direction and with the guidance of management. Organize recreational and other group activities at Mobile Unit locations. Develop outreach and educational presentations under the guidance of management.

Conduct educational presentations at various community locations to educate people about Independent Living and the services available through RCIL and CDR. Document all services and work activities in accordance with organizational, division and departmental expectations, policies, and procedures. Ensure compliance with the organization’s policy, and confidentiality standards. Write blog posts and newsletter articles on program activities. Participate in maintaining a neat, clean and safe work environment.

  • Excellent organizational skills and attention to detail.
  • Must be punctual, reliable, and able to respond quickly to consumer and organizational needs.
  • Reliable transportation and a valid driver license is required.

Qualifications
  • High School Diploma or General Education Degree required.
  • Associate’s degree with (3) to (5) years of relevant experience.
