CEC Health Care, a leading not-for-profit organization in Nassau County, is seeking a dedicated and compassionate Social Worker or Mental Health Counselor to join our Assertive Community Treatment (ACT) Team.
Our mission is to help adults diagnosed with serious mental illness achieve independence, self-determination, and wellness. As a member of our dynamic, multidisciplinary ACT team, you will play a vital role in delivering person-centered, community-based mental health services that empower clients to Aspire Higher.
In this role as an ACT Program Coordinator, you will:- Provide clinical and operational oversight to multiple ACT teams, ensuring compliance with OMH standards and fidelity to the ACT model
- Oversee Health Home program implementation, quality assurance, and performance improvement initiatives
- Conduct staff performance evaluations and support professional growth through coaching and mentorship
- Promote recovery-oriented, trauma-informed, and culturally responsive care
- Facilitate integrated care planning that incorporates behavioral, medical, and social service coordination
- Prepare and submit reports, documentation, and data to regulatory agencies and internal stakeholders
- Establish and maintain collaborative relationships with hospitals, primary care providers, housing agencies, and community-based organizations
- Participate in after-hours on-call rotation as assigned
- Master’s Degree in Social Work or Mental Health Counselor
- Current New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC)
- Minimum of three (3) years of progressive leadership experience in behavioral health settings
- Prior experience working in an ACT program strongly preferred
- Clean and valid NYS driver’s license and ability to travel throughout Nassau County
