The Medical Scribe supports wound care providers in skilled nursing facilities. The Medical Scribe will be responsible for accurately documenting patient encounters, ensuring timely and compliant entries into the electronic medical record (EMR), supporting Merit-Based Incentive Payment System (MIPS) reporting, and facilitating communication between the clinical team and facility staff. This role is essential in helping clinicians focus on direct patient care while maintaining high standards of documentation and regulatory compliance.
This role is onsite in Queens, NY.
Key Responsibilities- Accurately document patient visits, wound assessments, and treatment plans under the direction of the provider.
- Ensure all notes and entries are compliant with MIPS requirements and skilled nursing facility regulations.
- Assist with the preparation and organization of patient charts and medical records.
- Support the provider in capturing all relevant clinical data, including wound progress and procedure details.
- Communicate effectively with skilled nursing facility staff and administrators to help coordinate care and clarify documentation as needed.
- Monitor documentation timelines to ensure completion and submission in accordance with facility and payer guidelines.
- Maintain strict confidentiality of patient information and follow HIPAA regulations.
- Perform other duties as assigned to support the needs of the organization.
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Prior experience as a medical scribe, preferably in wound care, geriatrics, or post-acute settings.
- Strong knowledge of medical terminology, EMR systems, and healthcare documentation standards.
- Understanding of MIPS documentation and quality reporting preferred.
- Excellent written and verbal communication skills.
- Detail-oriented with strong organizational skills.
- Professional demeanor with the ability to work independently and as part of a clinical team.
