Location: New York
The Grand Rehabilitation and Nursing in Guilderland is now hiring a Director of Social Work (LMSW)
We are located in Altamont, NY - 25 minutes from Albany
The Director of Social Work oversees the Social Services department, ensures residents receive appropriate support, and guides the social work team in delivering person-centered care. This role focuses on leadership, resident advocacy, regulatory compliance, and coordination with interdisciplinary teams.
Key ResponsibilitiesDepartment Leadership & Oversight
- Supervise and support social work staff, ensuring high-quality services
- Establish department goals, policies, and workflow procedures
- Promote best practices and maintain a positive, supportive environment
Resident Support & Care Planning
- Assess residents’ social, emotional, and adjustment needs
- Develop and update individualized care plans
- Provide counseling, guidance, and assistance during transitions
Coordination & Communication
- Collaborate with nursing, medical, therapy, and administrative teams
- Communicate regularly with families regarding resident progress
- Participate in care plan meetings and interdisciplinary discussions
Compliance & Documentation
- Ensure adherence to state and federal long-term care regulations
- Oversee documentation, assessments, and required reporting
- Maintain confidentiality and uphold ethical standards
Community & Resource Engagement
- Connect residents and families with community service resources
- Represent the facility in occasional outreach or agency interactions
- Must be a LMSW (Licensed Master Social Worker)
- Experience in long-term care required
- Prior leadership or supervisory experience in a healthcare setting
- Strong communication, problem-solving, and organizational skills
- Knowledge of regulatory requirements in long-term care
- Ability to manage multiple priorities effectively
