Director of Social Work; LMSW

The Grand Rehabilitation and Nursing in Guilderland is now hiring a Director of Social Work (LMSW)

We are located in Altamont, NY - 25 minutes from Albany

The Director of Social Work oversees the Social Services department, ensures residents receive appropriate support, and guides the social work team in delivering person-centered care. This role focuses on leadership, resident advocacy, regulatory compliance, and coordination with interdisciplinary teams.

Department Leadership & Oversight

• Supervise and support social work staff, ensuring high-quality services
• Establish department goals, policies, and workflow procedures
• Promote best practices and maintain a positive, supportive environment

Resident Support & Care Planning

• Assess residents’ social, emotional, and adjustment needs
• Develop and update individualized care plans
• Provide counseling, guidance, and assistance during transitions

Coordination & Communication

• Collaborate with nursing, medical, therapy, and administrative teams
• Communicate regularly with families regarding resident progress
• Participate in care plan meetings and interdisciplinary discussions

Compliance & Documentation

• Ensure adherence to state and federal long-term care regulations
• Oversee documentation, assessments, and required reporting
• Maintain confidentiality and uphold ethical standards

Community & Resource Engagement

• Connect residents and families with community service resources
• Represent the facility in occasional outreach or agency interactions

• Must be a LMSW (Licensed Master Social Worker)
• Experience in long-term care required
• Prior leadership or supervisory experience in a healthcare setting
• Strong communication, problem-solving, and organizational skills
• Knowledge of regulatory requirements in long-term care
• Ability to manage multiple priorities effectively
