• Process, monitor, and review all reportable and non-reportable incidents in conformance with Part 624 of 14
  
  NYCRR.
• Conduct investigations as assigned by New York State Justice Center and Incident Management Coordinator.
• Serve on-call 24/7 for reporting and investigation of urgent agency incidents.
• Assist in staff training regarding incident identification and reporting.
• Interface with agency personnel and community/service providers.
• Attend monthly Incident Review Committee meetings and record accurate minutes.
• Generate meeting minutes/reports in a timely manner.
• Coordinate with Quality Assurance staff to ensure ongoing compliance with federal and state incident management requirements.
• Attend OPWDD facility audits when requested.
• Develop compliance protocols and administrative guidelines for the incident management process.
• Update the incident management database as necessary.
• Assist QA staff in implementing internal QA processes, as required.
• Perform additional duties as required.

• Bachelor’s degree required.
• Ability to multi‑task with a positive, pleasant disposition and meticulous attention to detail.
• Strict confidentiality and discretion required.
• Valid driver’s license.
• Excellent computer skills.
