Compensation Range: $65,000 - $70,000 yearly commensurate with experience.

OverviewUsing a one to one and group person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental health diagnoses, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning.
Responsibilities- Community Psychiatric Support and Treatment (CPST) Services:
Provide time-limited, goal directed interventions included in the ISP are time-limited, goal-directed support and solution-focused, with the intent to achieve stability and functional improvement. - Conduct Person-Centered Assessment, Service Planning, Documentation, and Collaboration. Provides Individual and Family Psychotherapy and Counseling and Integrated Co-Occurring Disorder Treatment. Actively engages around Crisis Prevention. Offers customized interventions for Intensive Rehabilitation counseling designed to intensely assist an individual in attaining and sustaining meaningful life roles.
- Psychosocial Rehabilitation (PSR) Services:
Engage in person-centered planning process. - Works to support the individuals' skill building and psychoeducation to develop or improve personal coping strategies, prevent crises, and promote recovery.
- Provides Rehabilitation Counseling to improve functioning in living, working, learning, and social environments.
- Maintain a caseload of clients, providing 25 hours of clinical care weekly on average.
- Assist clients in articulating personal goals using a client-centered approach.
- Demonstrated ability to work independently and as part of a team, including working collaboratively with professionals across disciplines (e.g., domestic violence advocates, housing specialists, attorneys, Rehabilitation counselors, government officials).
- The counselor will conduct intake assessment, counseling, advocacy, and referral services to HARP members.
- Opportunity to receive training in evidence-based modalities.
- Hybrid work requirement with both remote and in-person formats with a majority of work expected to be done in community with the clients.
- Provide services as outlined in each participant's Plan of Care.
- Assist clients with referral to other services and agencies necessary to the recovery process (e.g., shelters, detox units, legal counsel).
- Collaborate with clients’ care teams and providers to determine the steps needed to achieve individuals' goals and self-directed recovery.
- Teach clients how to identify and combat negative self-talk and how to identify and overcome fears.
- Support clients’ vocational/educational choices and assist them in choosing a job that matches their strengths, overcoming job-related anxiety by reviewing job applications, and providing interview tips.
- Maintain case records including progress notes and Individualized service plan in compliance with state regulatory requirements.

Other duties as assigned. - Masters Degree in the Healing Arts with active license in one of the following to practice is required:
Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), Licensed Creative Arts Therapist (LCAT). - Experiences with serving people with mental health diagnoses or substance use preferred.
