Position Summary

We have an exciting opportunity to join our team as a Program Manager, Pharmacy Operations (Part-Time - 17.5).

The Program Manager, Pharmacy Operations is a Clinical Pharmacist who provides comprehensive medication management of patients with Non‑Tuberculous Mycobacteria (NTM) infection, Bronchiectasis, and Cystic Fibrosis (CF) in the Division of Pulmonary, Critical Care and Sleep Medicine outpatient practice. This part‑time role is essential in optimizing therapeutic outcomes, minimizing drug‑related problems, and ensuring safe and effective use of complex medication regimens for this unique patient population.

The Program Manager develops and implements policies and procedures to optimize workflow and ensure patient satisfaction; partners with the Directors of the programs to lead program initiatives and quality assessment and performance improvement activities; coordinates with multidisciplinary ancillary staff; maintains familiarity with local and federal regulations for advanced therapies for diseases, advances the research productivity of the team, and assists with all of the programs marketing and outreach pursuits.

Job Responsibilities Direct Patient Care Responsibilities
  • In collaboration with physicians, clinical team members and program directors, coordinates all pharmacotherapy needs patients in the outpatient practice.
  • Maintains contact with assigned caseload between visits, based on their particular needs, and initiates appropriate responses to issues and problems during these contacts.
  • Communicates with the multidisciplinary team to optimize patient care. Interfaces with physicians, social workers, nurses, nurse practitioners, dietitians, and others to plan and execute specialized protocols.
  • Informs physicians of patient status and needs and monitors clinical patient indicators to coordinate care with the patients physician.
  • Provides patient and family education and excellent patient services to ensure satisfaction.
  • Develops and maintains a high‑level clinical pharmacy practice. Participates in multidisciplinary, bedside rounds. Provides pharmacotherapeutic, pharmacokinetic and pharmaco‑economic consultation.
  • Provides patient and caregiver medication counseling and participates in educational initiatives.
  • Develops and implements clinical practice guidelines that will enhance pharmaceutical care and medication safety.
  • Documents and reports clinical pharmacy interventions, adverse drug reactions, and medication errors.
Education, Development and Research
  • Promotes own professional growth and development in clinical and managerial role.
  • Serves as a clinical expert and resource for the education of peers, staff members and other health professionals.
  • Participates in educational and outreach programs for patients and their families, and other health professionals.
  • Assists in the preparation, implementation, and evaluation of research protocols when applicable.
  • Moves forward the research productivity of the team and takes lead in investigator‑initiated trials.
Minimum Qualifications

Licensure &

Education:

Doctor of Pharmacy degree from an ACPE accredited school of pharmacy. Active New York State Pharmacist Licensure. PGY1 pharmacy residency training. PGY2 pharmacy residency training or equivalent salaried professorship at a school of pharmacy.…
