Prevention Social Worker / Mental Health Counselor

New Alternatives for Children, Inc. (NAC) is an award‑winning not‑for‑profit agency in Midtown Manhattan dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs.

The Special Medical Prevention Services program seeks a Prevention Social Worker / Mental Health Counselor to provide clinical, case‑management, and supportive services to families who are child‑welfare involved and caring for children diagnosed with chronic or complex medical conditions.

The ideal candidate will be strength‑based, trauma‑informed, culturally responsive, and committed to family preservation and resilience. This city‑wide home and community‑based program requires the ability to work independently and flex hours on weekdays to meet the needs of youth and caregivers. The hybrid model supports work‑from‑home when staff are not in the field.

Description of Responsibilities
  • Provide casework and clinical services to youth and caregivers using individual and family modalities, coordinating services for a caseload of 8‑9 families as recommended by NAC’s multidisciplinary treatment team.
  • Conduct home and community‑based visits.
  • Adhere to agency and ACS regulatory requirements related to casework and collateral contacts, Family Team Conferencing, Workforce Institute training, and timely completion of Family Assessment and Service Plans (FASPS) and progress notes in CONNECTIONS.
  • Utilize individual, group, and peer supervision, clinical consultation, mental‑health in‑services, and social‑justice discussion forums. Candidates receive credit toward NYS LCSW and LMHC licensing requirements. Free CEUs are available to licensed social workers (LMSW/LCSW) and licensing prep courses for MSWs and MHCs who have completed probation.
  • Collaborate with psychiatrists, psychologists, pediatricians, nurses, case associates, and resource/education specialists in a dynamic, strength‑based interdisciplinary team model.
Required

Education and Experience
  • Master’s degree in social work, mental health counseling, public administration, psychology, or a related field.
  • Prior experience in preventive services, child welfare, and/or health care is preferred.
Required Skills
  • Excellent written and verbal communication and organizational skills.
  • Strong engagement skills.
  • Self‑awareness and motivation for team building.
  • Proficiency in Spanish is preferred and includes a salary differential.
