The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery.

The Social Care Network Screener/Navigator serves as an entry point for the delivery of HRSN services, engaging Medicaid participants to identify health-related social needs and connect them with appropriate community-based care. This position is responsible for screening individuals, assessing NYS Medicaid eligibility for HRSN and enhanced-care services. The Screener/Navigator guides participants through the process of accessing support resources. The SCN Screener/Navigator will cover multiple EA sites (Manny Cantor and Sirovich centers).

Responsibilities
  • Conduct outreach to NYS Medicaid participants to identify unmet social needs.
  • Provide education on benefits and community supports available under the NYS Medicaid program.
  • Screen individuals for health-related social needs using standardized tools.
  • Assess eligibility for enhanced care services and community-based programs.
  • Develop care management plans as applicable.
  • Provide navigation support to connect individuals with appropriate services.
  • Collaborate with care teams and community partners to ensure coordinated support.
  • Document interactions, referrals, and follow-ups accurately in case management platform.
  • Maintain HIPPA, 42

    CFR-part 2 confidentiality standards, and act with cultural sensitivity and empathy.
  • Develop relationships with community groups/agencies in partnership with the Center.
  • Adhere to agency, NYS Medicaid, and program-specific policies and procedures.
  • Attend staff meetings, trainings, and other pertinent events as recommended by supervisors.
  • Perform any other duties as required.
Requirements
  • High school diploma or equivalent required, Bachelor preferred.
  • Experience in social services, community outreach, or patient navigation.
  • Knowledge of social care systems, local resources, and health-related social need principles.
  • Familiarity with NYS Medicaid eligibility, benefits, and referral systems.
  • Excellent communication, interpersonal, and active listening skills.
  • Technological proficiency with data entry, recordkeeping, and screening tools.
  • Ability to work independently and as part of an interdisciplinary team.
  • Some local travel is required for community site visits or events (monthly Metro Card / equivalent will be provided).
  • Must maintain professionalism in sensitive and confidential situations.
  • Flexibility as the agency rolls out enhanced care services.
