The Community Health Worker (CHW) engages one‑on‑one with clients and their families and social support networks to support the client’s engagement with their health goals and priorities through community outreach, home visits, navigation, ongoing communication and advocacy. The CHW works primarily in community‑based settings as an extension of the hospital or clinic.
Essential Functions- Deliver patient outreach, education, advocacy and support in community settings to close care gaps.
- Maintain a client caseload as determined by the Population Health department.
- Assist clients in homes, community or clinic settings to address health‑related social needs and improve engagement with care management plans.
- Help clients identify and prioritize health and socio‑economic issues and develop health/social management plans and goals.
- Foster partnerships with community‑based organizations, health systems and community stakeholders to establish referral pipelines for clients.
- Create action‑plan reinforcement sessions to help participants achieve health goals.
- Connect residents to social services and support (Health Insurance, SNAP, legal aid, etc.) by facilitating appointments and accompanying them when needed.
- Accompany clients to medical and social service visits as required.
- Prepare case‑related reports, including outcomes and successes.
- Conduct basic health assessments, screenings and care plans.
- Distribute outreach materials such as fact sheets and flyers to promote the program in targeted areas.
- Utilize community engagement and recruitment strategies.
- Participate in health fairs and community events to promote services and increase program visibility.
- Educate clients on community organizing and mobilization basics.
- Promote leadership development and wellness capacity building for community members.
- Facilitate and/or support educational workshops and group wellness activities.
- Track all required outcomes using the program data system, Excel and other software.
- Actively participate in case conferences and meetings.
- Attend required training and maintain current knowledge of program information.
GED, High School Diploma, or equivalent.
ScheduleMonday‑Friday, 9 AM‑5 PM with options for some remote access.
QualificationsRequired:
High School diploma or GED. Preferred:
Experience in community health, social services or related field; knowledge of community resources and referral networks.
