Under the direction of Life Coach V and Life Coach X, this position maintains an environment in the Residential programs that encourages optimal personal development, provides person-directed services, and ensures the health and safety of the individuals served.
Job Functions Regulatory and Corporate Compliance- Ensure compliance with regulations, agency policies and procedures.
- Ensure required documentation is complete, accurate and legible.
- Ensure the agency’s incident management process is followed.
- Provide input in the development and revisions of policies, procedures and agency forms as needed.
- Ensure chain of command is followed when necessary.
- Promote the highest quality of support by implementing the Code of Ethics and Core Competencies.
- Keep all information confidential.
- Assist with ensuring adherence to the annual budget.
- Assist with ensuring accurate management of individuals’ funds.
- Complete personal allowance documentation and other financial documentation according to procedure.
- Provide services as planned in individuals’ Individual Service Plans (ISP) and related documents.
- Act as a Resident Advocate to ensure ISPs, IPOP, PONS and Behavior Plans are accurate.
- Implement activities outlined in Habilitation Plans and ISP.
- Plan and coordinate daily activities.
- Advocate for individuals.
- Provide person-directed services, including residential or day habilitative services and assistance in personal skill areas.
- Complete assessments and compile baseline data as assigned.
- Assist and support individuals with community integration.
- Respect individuals’ right to privacy and confidentiality.
- Work collaboratively with others to ensure health and well‑being.
- Ensure emergency medical procedures are followed and consult with the medical chain of command when necessary.
- Provide services and follow individuals’ Plan of Nursing Services (PONS).
- Report and document all medical concerns to the appropriate personnel.
- Transport and attend health and medical appointments as assigned.
- Complete weekly one‑on‑one meetings with Life Coach V.
- Attend staff meetings and provide updates.
- Complete one‑page profiles and assist POMS as identified.
- Maintain an environment that encourages optimal personal development.
- Complete assigned daily responsibilities and shift overlaps.
- Handle documentation, filing, and general office maintenance.
- Assist in identifying and developing new services and support opportunities.
- Complete monthly summaries if applicable.
- Comply with minimum staffing policy.
- Ensure the site is a safe, clean environment and address concerns immediately.
- Maintain knowledge of fire evacuation plans.
- Practice standard precautions and infection control practices.
- Ensure site vehicles are maintained when applicable.
- Develop and maintain positive customer relationships internally and externally.
- Maintain effective working relationships with service providers, community resources, and families.
- Ensure internal and external communications reflect agency’s mission, vision, and values.
- Address customer concerns timely and professionally.
- Participate in agency committees, initiatives, projects, and trainings.
- Assist with coverage as assigned.
- Maintain certifications and annual training updates.
- May use private vehicle to transport people served.
- Flexible with work locations and hours, including any residential home.
