Clinical Social Worker (LMSW/LMHC/LCSW) – Corona, Queens

Join URBAN HEALTH PLAN, INC. as a Clinical Social Worker in our Corona-Queens Family Health Center.

We are expanding services at Plaza del Sol, opening a new floor. We are seeking Clinical Therapists (LMSW, LMHC, or LCSW) to provide clinical assessment and individual therapy for patients.

• Provides bio‑psychosocial assessments and clinical diagnostic assessments.
• Conducts individual psychotherapy and counseling services to children in public schools.
• Maintains skills and knowledge in clinical theory and practice to help patients overcome dependencies, adjust to life, and make changes.
• Provides counseling to family members to assist with understanding and supporting patients.
• Engages in crisis intervention when necessary and refers patients or family to community resources.
• Provides consultation to medical providers and collaborates with physicians to develop and implement treatment and intervention programs.
• Participates in clinical team meetings.

• Master's Degree in Social Work from an accredited institution.
• NYS Licensed LMSW, LMHC, or LCSW.
• 1–2 years of experience providing counseling.
• Bilingual Spanish/English.
• Ability to thrive in a fast‑paced environment as a member of an interdisciplinary team.
• Ability to identify high‑risk psychosocial factors in patients and families that impact health status.

LMSW/LMHC:
Minimum $72,000 per year;
Maximum $80,000 per year. LCSW:
Minimum $85,000 per year;
Maximum $94,000 per year.

Onsite, Corona, Queens, Monday to Friday 8:00 am–5:00 pm.
