Provides physical and behavioral health care coordination and promotes integrated medical and psychiatric care for ACT recipients with severe mental illness and/or substance use conditions.

• Provides direct, face-to-face treatment and health screenings in the community.
• Guides team on preventative and critical health matters including nutrition.
• Administers injections and educates on medication adherence.
• Collaborates with psychiatrist and team on conditions and care delivery.
• Coordinates pharmacological services, labs, and referrals to providers.
• Implements wellness programs, health action plans, and risk assessments.
• Supports treatment planning and person-centered recovery goals.
• Provides staff training and education on health care integration.
• Completes all assessments, reports, and documentation on time.

• Licensed by NYS Education Department.
• 3 years post-license experience treating individuals with mental illness.
• 12 years wellness/health program experience preferred.
• Strong organizational, communication, and critical thinking skills.

• Transport clients as needed.
• Travel to client homes and agency sites.
• Availability 8 AM-8 PM with on-call support after hours.
• Direct in-person interaction required.

• Background check/fingerprinting required.
• Valid driver's license and car insurance required.
