• Daily personal care (bathing, grooming, oral care, dressing, etc.)
• Assist with daily living activities per the Plan of Care
• Monitor patients and report on condition
• Help with patient meals and feeding, and basic housekeeping functions
• Accompany patient to doctor visits, shopping, and other errands

• Must have New York State Home Health Aide Certificate
• Government‑issued , Social Security Card, and work authorization if needed
• Two reference letters
• Recent physical exam
• TB screening (Quantiferon preferred)
• Rubella & Rubeola titer lab work

• Seniority level: Entry level
