Location: New York
Overview
DSP/Medical Assistant, Full-time (40 hours), 6:00am – 2:00pm, Monday–Friday (Morning). The role is with The Salvation Army.
Responsibilities- Provides and ensures a safe, clean, and home-like environment; informs Program Manager of Physical Plant needs.
- Promptly informs the Program Manager/Assistant Program Manager of any serious incident or violations of resident and/or staff rights, according to established policies.
- Develops Activities of Daily Living (A.D.L.) and Independent Living Skills (I.L.S.) for residents as it relates to needs and treatment goals.
- Maintains knowledge of each resident’s medical, psychological, cognitive, and social needs; ensures programming aligns with needs and treatment goals.
- Maintains and submits timely and accurate records and reports (daily goal data, monthly progress notes, summaries, etc.).
- Maintains and checks Medication Administration Records daily as needed; orders medications per doctor’s orders and schedules medication appointments.
- Reviews residents’ medical records to ensure follow-up appointments are completed; communicates with nurse and management about appointments/follow-ups.
- Participates in interdisciplinary treatment meetings and regularly scheduled resident reviews.
- Provides habilitation services as prescribed in each client’s service plans (CFA, Individual Service Plan, Habilitation Plan).
- Assists in transporting individuals to programs, community activities, medical appointments, and home visits.
- Assists clients in skill development in daily living, domestic living, recreation, and socialization.
- Reports any deficiencies in habilitation services or safety issues to management; reports abuse and facility problems as required.
- Ensures residents’ nutritional needs are met and assists with feeding as necessary; assists with meal preparation as needed.
- Manages agency/resident funds as assigned for purchases; escorts individuals to medical appointments and completes required forms.
- Communicates with Program Nurse and supervisors about residents and program issues to ensure continuity of care.
- Maintains knowledge of residents’ medical appointments and follow-up needs; collaborates with housekeeping/cook on facility maintenance as needed.
- Adheres to essential functions of the job as listed in the description and follows attendance standards per supervision.
- Completes medication certification and maintains proficiency; performs housekeeping duties as assigned; and participates in other duties as required by the Program Manager.
- Demonstrates awareness of cultural and socioeconomic characteristics of the served population.
- High School Diploma or GED required;
Associate’s Degree preferred; minimum of one year’s experience in Human Services. - Excellent oral and written communication skills; ability to learn and use computer systems; strong teamwork and professional conduct.
- Experience in medical care and interaction with medical professionals.
- Must pass AMAP certification training and maintain valid certification throughout employment.
- Valid Driver’s License; ability to drive a large vehicle (wheelchair or 15-seated van).
- SCIP R and CPR & First Aid required.
