This position offers an excellent opportunity for an experienced human service professional to play a pivotal role in changing the life trajectory of New Yorkers who have faced homelessness, violence, trauma, unmanaged behavioral health conditions, incarceration, discrimination, and extreme poverty.

• Serve as a clinical staff member, providing individual, group, and family therapy to help clients process trauma and adopt behavioral strategies that support recovery and healthy relationships.
• Conduct immediate needs assessments, develop written plans, and prioritize client needs with the multidisciplinary ACT team.
• Participate in in‑depth comprehensive assessments, dynamic care planning, and routine team duties to implement care plans.
• Utilize Cognitive‑Behavioral strategies to shift thinking and behavior, coping with common stresses and emotional triggers.
• Engage in community outreach, periodic on‑call coverage, and flexible hours in a field‑based environment.
• Document all interventions, attempted contacts, and service updating in the EHR in accordance with program standards.
• Provide comprehensive transitional care following hospitalization events in accordance with ACMH Critical Time Intervention (CTI) protocols.
• Facilitate crisis intervention and care delivery by scheduling appointments, obtaining necessary information, and arranging transportation.
• Attend in‑service training, team meetings, and weekly clinical conferences as required.

Master’s degree in social work, mental health counseling, nursing, or psychology and at least one year of direct service experience in the human services field, nursing, or Care Management/Service Coordination. Demonstrated competency in clinical documentation, Motivational Interviewing, Suicide Assessment/Prevention, Trauma‑informed Care, Psychosocial Rehabilitation Principles, cognitive‑behavioral strategies, and stage‑wise treatment. Strong written and verbal communication skills. Licensure required: LMSW, LCSW, LMHC, or limited permit.

$71,438 plus generous benefits.
