Guild Care, Lighthouse Guild’s innovative community-based Adult Day Health Care Program providing services to adults who are blind or visually impaired and have chronic health care needs is seeking a Teacher/Instructor Teacher-Level I Communication Skills for its Manhattan location.
Responsibilities:
- Completes vision rehabilitation assessments for each registrant in the areas of communications skills, orientation and mobility, and activities of daily living.
- Develops and implements vision rehabilitation treatment plans for those registrants needing vision rehabilitation training at the Guild Care site. Re-evaluates needs periodically and revises treatment plans as needed.
- Teaches communication skills, home management skills, personal care skills and orientation and mobility following the established Guild Care Core Curriculum for Vision Rehabilitation.
- Works with individuals and groups.
- Serves as resource person with respect to services, equipment, supplies and materials for the visually impaired.
- Adapts materials for individual clients.
- Provides liaison with and makes referrals to appropriate community agencies.
- Participates in team development of daily living, educational cultural and recreational program for registrants.
- Completes all required records and reports on a timely basis.
- Works with other team members to foster interdisciplinary understanding, cooperation and a unified treatment approach.
- Along with all other Guild Care staff, performs general assistance tasks on a regular basis as assigned by program director. These include: escorting registrants throughout the space, including to from the restroom, assisting with transportation, serving meals cleaning up from meals as needed, and filling in for absent coworkers.
- Observes and implements the Registrant's Bill of Rights.
- Responsible for maintaining the privacy of registrant protected health information.
- Responsible for following the LGI Workforce security policy; will access information systems and ePHI authorized by Clinical Authorization only.
- Standing for long periods, walking, lifting/transferring registrants, using lifting equipment when required.
- Bending, stooping, squatting, twisting, reaching above and below shoulder level.
- Able to push/pull registrants in wheelchairs.
- Able to lift and carry equipment and/or supplies
Qualifications:
A Bachelor's Degree is required. Experience teaching the visually impaired or willing to work with VI instructors and learn.
Accountability:
The Teacher/Instructor Level I is responsible to the Guild Care Program Director.
