About The Role
Doral Health & Wellness is seeking a detail-oriented and compassionate HCBS Coordinator to support clients enrolled in Home and Community-Based Services (HCBS). This role focuses on care coordination, Medicaid compliance, documentation, and communication with clients, families, and service providers to ensure high-quality community-based care.
Key Responsibilities

• Coordinate HCBS services and supports for enrolled clients
• Monitor client progress and ensure individualized service plans are followed
• Communicate with clients, families, and service providers
• Maintain accurate documentation in EHR/case management systems
• Assist with scheduling appointments and community services
• Support compliance with Medicaid and HCBS regulations

• High school diploma required;
  Bachelor’s degree preferred
• Experience in HCBS, Medicaid, case management, or community-based services preferred
• Strong organizational, communication, and time-management skills
• Familiarity with EHR or care coordination systems

• Salary: $75,000 – $85,000
