DHCare Licensed Home Care Agency is seeking an experienced Bilingual Spanish-Speaking Intake Specialist to join our Intake Department. We are a home care agency providing PCA and CDPAP services to the community. The ideal candidate will play a key role in managing referrals and ensuring a smooth transition from intake to patient activation.
Responsibilities- Receive and review patient referrals
- Follow up with patients and MLTCs
- Coordinate and ensure smooth transition from referral to patient activation
- Maintain accurate and timely communication via email and phone
- Bilingual in Spanish and English (Required)
- Proficient in email communication and Microsoft Office applications
- Strong verbal and written communication skills
- Detail-oriented with excellent time management and organizational skills
- Strong interpersonal and relationship-building abilities
- High school diploma or equivalent (Preferred)
- Customer service experience: 1 year (Preferred)
- Computer skills: 1 year (Preferred)
- Health insurance
- Paid time off
- Ability to relocate to Jamaica, NY 11432 before starting work (Required)
- Pay: $22.00 - $25.00 per hour
