Location: New York
A rapidly growing home healthcare provider is seeking a motivated and compassionate Patient Coordinator to support patient relations, satisfaction, and retention. This role involves building strong, trusting relationships with patients and caregivers while helping ensure they receive consistent, high-quality care. The ideal candidate is empathetic, proactive, and highly communicative, thriving in a role that combines customer service, follow-up, and problem-solving.
Responsibilities- Conducting follow‑up calls to patients and caregivers
- Addressing service concerns and resolving issues promptly
- Building ongoing relationships to support patient retention
- Coordinating with internal teams to ensure seamless care delivery
- Tracking patient interactions and documenting communication
- Assisting with general patient support as needed
- Strong communication and interpersonal skills
- Empathetic, patient‑focused, and professional
- Detail‑oriented and organized
- Experience in healthcare coordination or customer service preferred
- Home care experience and bilingual English/Spanish are strong pluses
$23 - $25/Hour
