Position: Certified Peer Specialist, Critical Time Intervention (CTI)

Function :
Serves as a member of the CTI Team, providing supportive counseling and time-limited intensive care coordination for a caseload of high-risk clients. Responsibilities include promoting the development of skills for coping with and managing behavioural health symptoms, providing linkage to resources, and engaging individuals in skill building.

Reports to: Team Leader, Critical Time Intervention

Location: Manhattan

Schedule: Monday – Friday (9:00 am–5:00 pm)

Tasks:

  • Responsible for engaging clients at hospital bedside to provide program overview and conduct intake assessments.
  • Develops rapport with clients in order to engage them in improving their health and wellness.
  • Participates in weekly team meetings to discuss new referrals and provide updates on enrolled clients.
  • Develops WRAP and Relapse Prevention Plans to help clients understand their patterns and triggers for use and identify interventions to deal with triggers and cravings and identify supports.
  • Acts as a role model to educate client about self‑help techniques and self‑help group processes, teaches effective coping strategies and teaches symptom management skills.
  • Provides transportation training and escorts to appointments.
  • Assists clients in the development of community support systems and networks.
  • Provides hands‑on assistance with activities of daily living.
  • Provides training in self‑advocacy.
  • Supports client goals and serves as an advocate on client’s behalf.
  • Identifies, facilitates and secures access to needed healthcare, social services benefits, and community resources. Works in collaboration with other care team members and care providers, including behavioural health, disease care management, home care, social work and community‑based organisations, to help client achieve optimal health outcomes.
  • Embraces the team model by collaborating with members of the team and providing support as needed.
  • Utilises evidence‑based practices, such as motivational interviewing, to empower clients to grow and attain goals.
  • Implements tasks outlined on the care plan and ensures follow‑up and continuity of care between client interactions.
  • Documents all interventions and attempted contacts in the EHR in accordance with programme standards.
  • Assesses domiciled client’s living conditions by conducting home visits.
  • Attends in‑service training as requested.
  • Duties as assigned by supervisor.

Requirements:

Lived experience receiving mental health services and a commitment to assist others in attaining recovery and wellness goals. High School diploma or G.E.D. Good written and verbal communication skills in English. Basic computer and data entry skills. Must be a Certified Peer Specialist with New York State.

Salary: $52,633 plus generous benefits.
