Mount Sinai - 1176 FIFTH AVE (KP), 9, [Medical Office Assistant / Scheduler] As a Patient Navigator at Mount Sinai, you'll:
Carry out tasks to execute the medical and support service plans including guiding patients to appointments, and accompanying them when necessary;
Ensure that medication adherence issues are addressed;
Act as the primary liaison with medical providers to ensure patient adherence;
Deliver monthly or weekly health education encounters;
Document all patient encounters and enter all data into web-based system;
Coordinate logistics for plan adherence.
