The Cottages at Garden Grove is looking for motivated
* Speech Therapist
* to join a great team in our person centered home environment. The Cottages at Garden Grove is set on a lovely campus in Cicero, NY and is made up of 12 small-home residences. This model of small-homes sets a new standard for skilled nursing and short-term rehab in CNY. These homes come complete with a sense of neighborhood and welcoming atmosphere that will make everyone who resides and works here feel at home.

Therapists in Clinical Fellowship Year (CFY) encouraged to apply.

* Job Duties
* * Evaluates and treats patients with different diagnosis in accordance with physician's orders.

* Initiates skilled speech therapy treatments in accordance with physician's orders.

* Provides skilled speech therapy through the application of principles, methods and procedures of measurement, prediction, non-medical diagnosis, testing, counseling, consultation, rehabilitation and instruction related to the development and disorders of speech, voice, swallowing and/or language.

* Completes and maintains required documentation such as evaluations, progress notes, daily notes, recertifications, and discharge summaries for patients on caseload.

* Identify and meet patients' goals and needs.

* Offer cost-effective treatments that help improve patient' speech, voice, swallowing and/or language.

* Develop care plans using a variety of treatment techniques.

* Screens patients for communication and provides audiometric screening and makes referrals for further evaluation if necessary.

* Provides quality, personalized and evidence-based care, and proven interventions.

* Motivates patients during treatment to help them function optimally.

* Consults and practices with other health professionals regarding special needs of patients to enhance rehabilitation or maintenance of function.

* Provides input for the Care Planning meetings and attends during absences of the DOR.

* Maintains statistical records of residents' treatments.

* Assists with in-service training programs upon request.

* Submits requisition for supplies needed in the treatment of residents to DOR.

* Reviews written speech-language policies and procedures annually and makes necessary revisions.

* Observes and practices confidentiality of resident information and privacy when performing job duties.

* Recognizes and follows the dress code of the facility including wearing name tag at all times.

* Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.

* Attends facility meetings, staff meetings, and required or preferred in-services.

* Generates and develops ideas which improve the quality of care for patients in the facility.

* Performs all position duties according to safety rules and practices required for this position, and as required to ensure the general safety of staff, residents and visitors of this facility.

* Demonstrates knowledge and understanding of all policies and procedures and ability to reference them from appropriate manual.

* Exhibits the highest level of customer service towards residents, staff and visitors.

* Ability to work with the highest efficiency and time management.

* Exhibits flexibility and consistently demonstrates the ability to respond to changing situations to meet facility and patient needs.

* Follows policy and procedure regarding all electronic devices, computers, tablets, etc.

* Supports and abides by the facility's Mission, Vision, and Values.

* Abides by the facility's businesses code of conduct, compliance and HIPAA policies.

* Performs other duties as assigned by the supervisor, management staff or Administrator.
